Easy Mixer Design?

Hey there guys.. so i'm laying out a design for my diy case for a mixer.
What i would like is 4 channels... each with level, pan and kill switch

Stereo out which is the sums of the panned stages.... and level over master out.

Nothing crazy... suppose I could use the cgs dc mixer (already have one that i can hijack) but can someone knock it into my skull how i would wire for panning and how to get the 4 inputs to designate to L AND R respectively?
Maybe i would need a 2nd board or something? Thanks guys!! :)

The CGS81 is a simple stereo panning mixer.
