help wiring power one supply

help wiring power one supply

So i just got a power one hbb15 and although it seems somewhat obvious as to how to wire i am looking for instructions from somebody who has already done so. I am new to this and don't want to do something incorrectly.

The outputs seem self explanatory (- out/+out/common) but the input is a little confusing. on the case it says apply ac on pins 1+5 @100 and 1+4 @ 120.
i live in the u.s. and am planning on only powering @ +/- 15 v.

Also, any recommendations on fuses and a switch would be greatly appreciated.

The US is 120VAC nominal, which is usually 110 at the wall. Go with 1&4
